Brown Chiropractic: Online Appointment Manager Terms of Use
By using the "Online Appointment Manager" you, the user, agree to all of the following terms:

The "Online Appointment Manager" is offered as a free service to patients of Brown Chiropractic. It is
intended as a convenience to allow individuals to manage appointment scheduling when their personal
schedule permits.
It is not intended to be a stand alone appointment scheduling service. No
guarantees are made regarding appointment availability or to the function of the service. You are
merely REQUESTING an appointment day and time with this service. Until Dr. Brown or a staff member
contacts you to confirm you DO NOT HAVE A SCHEDULED APPOINTMENT!
Users of the Brown Chiropractic
Online Appointment Manager service do so at their own risk. By using the service the user(s) agree to
hold harmless James R. Brown from any and all damages and/or liability which might arise from the use
of the service.

Although every reasonable precaution is taken to protect your information, Brown Chiropractic and
James R. Brown, D.C. make no guarantees regarding the security of the service. Since no protected or
private health information is collected the Online Appointment Manager is not required to be deemed
"HIPAA Compliant".

Any appointments scheduled, edited, or modified with the Online Appointment Manager are subject to
any all applicable fees, office procedures, and policies of Brown Chiropractic. Missed appointment fees
and the required advance 24 hr notice of appointment cancellation etc. are just a couple of examples.
A reasonable amount of time must exist between the time you make your request and the requested
appointment time. We determine a "reasonable amount of time" to be no less than a 24 hour notice for
scheduling existing patient appointments. Weekend Appointments, New patients, and nutritional
consultation appointments will likely at least a 24 hour and at times up to a 48 hr advance notice.

Brown Chiropractic will never utilize your personal information for marketing purposes without your
consent. Brown Chiropractic Privacy procedures listed in our "Notice of Privacy Practices" apply to
information collected with the Online Appointment Manager. You will be/were provided a copy of this
Notice at the start of your care at Brown Chiropractic. If you need another copy CONTACT US and we
will be happy to provide one.

Problems with electronic services can and do occur. The safest and most reliable way to schedule an
appointment is always in person or live via the telephone. You WILL receive either a telephone
confirmation, email confirmation or BOTH for your requested appointment. If you have not received a
confirmation within 12 hours of your appointment please contact us. Until you receive some sort of
confirmation you should assume your appointment DOES NOT EXIST. Brown Chirorpractic and James R.
Brown, D.C. can accept no responsibility for problems arising from technical problems related to the
use of the Online Appointment Manager. Server problems, power failures, lost, misdirected email,
computer damage and other technical problems are not an acceptable excuse for waiving fees or
normal policy or procedure at Brown Chiropractic. James R. Brown, D.C. reserves the right to modify
these terms in unusual circumstances and on a case by case basis.

If you do not agree with any of the above please do not use the "Online Appointment Manager".
